Reporting a Problem

Has something happened and you’re not sure what to do next?  Below is some information on what to do and where to go for assistance.    

Do you believe you were treated unfairly by a professor? 

• Complete the grievance form.

Are you a community member wanting to report an incident either on campus or in Bloomington-Normal? 

• Complete our public incident report form.

Do you want to report a problem with a student organization? 

• Complete the public incident report form

Are you a student wishing to file a complaint against another student? 

• Call the office at (309) 438-8621 or e-mail us at to schedule an appointment with a staff member to discuss your options.

Has a crime occurred? 

• If so, contact Illinois State University Police (if it happened on campus) or Normal Police (if it happened in the Town of Normal).

Does the incident in question involve behavior that might be considered racial or sexual harassment/discrimination? 

• If so, report the incident to the the Office of Equal Opportunity, Ethics and Access at (309) 438-3383 for an investigation. 
• Reporting Acts of Intolerance/Bias and Reporting Criminal Acts - Important information regarding the reporting of biased or criminal acts.
