Has a crime occurred?
If so, contact Illinois State University Police (if it happened on campus) or Normal Police (if it happened in the Town of Normal).
The Campus Safety and Security website has more important information regarding the reporting of a crime.

Are you reporting an incident involving a student or a registered student organization?
Please complete our public incident report form.

Are you an instructor and need to report academic dishonesty?
Please complete the academic integrity reporting form.
If you need assistance completing the form, please review the instructions beginning on page 7 in the Faculty Guide for Academic Integrity.

Does the incident in question involve behavior that might be considered racial or sexual harassment/discrimination?
If so, report the incident to the Office of Equal Opportunity and Access (OEOA) by calling (309) 438-3383.
For incidents involving hate or bias, the Inclusive Community Response Team may also be contacted in addition to OEOA.
